Head Start / Early Head Start  Enrollment

Qualifications:
 
  • Age :  Between 2 weeks and 3 years for Early Head Start.
  • Age :  Between 3 years and 5 years for Head Start.
  • At least 10% of enrollment opportunities are for children with disabilities.
  • Expectant Mothers can begin the application process prior to the delivery date.

Requirements:
These documentation items must be provided.
 
  • Proof of income (copy of last year's tax return, paycheck stubs, TANF letter, child support documentation, etc.)
  • Current immunization record for child
  • Work and / or school schedule for parents
  • Certified birth certificate for child
  • Social Security Card for child (if applicable)
  • Medical insurance card through Sooner Care or private insurance
  • If your child has been diagnosed with a disability, including speech / language disorders,  attach a copy of all available reports, evaluations, and current I.E.P. / IFSP.
