What is Title XX ?

Crossroads Youth and Family Services offers full day services through our Extended Learning Program to working parents and parents attending school.  Once a family qualifies for child care subsidy, OKDHS will pay part or all of their child's extended day payments.
   
     
What are the Requirements ?
 

Applicants interested in child care assistance must apply at their local DHS office.  You will need the Crossroads HS / EHS Contract Number in order to ensure that you get approved for our site.  Crossroads' HS / EHS contract number is available through your local Head Start / Early Head Start Center. You can also refer to the OKDHS web page for information on the application process.

Once approved for Child Care subsidy, you will be responsible for swiping your child in and out of their center daily.  If you are assigned a family share co-payment, you will need to pay that by the 15th of each month at your youngest child's center. 
 

 
     
How do I apply ?    Summary
  1. Fill out "Request for Benefits and Services" form. Get the OKDHS form here.  Instructions are included on the OKDHS request form.
  2. Get the application to the local OKDHS office along with any documents required by DHS.
  3. Approval or Denial letter will be returned to you.
  4. If Approved by OKDHS, you may bring the approval letter to Crossroads HS / EHS for processing.
